In brief

The FDA approved ensitrelvir (Xocova) in June 2026 for post-exposure prophylaxis of COVID-19 in individuals aged 12 and older. This approval follows a phase 3 trial that demonstrated a 67% reduction in symptomatic infection risk when administered within 72 hours of exposure. Ensitrelvir is the first oral antiviral authorized for this purpose in the United States.

The U.S. Food and Drug Administration (FDA) has approved ensitrelvir (Xocova) as the first oral medication for post-exposure prophylaxis (PEP) of COVID-19 in adults and adolescents aged 12 years and older. This approval, announced in June 2026, marks a significant step forward in preventing symptomatic COVID-19 infections after exposure to the virus. Developed by Shionogi, ensitrelvir offers a new option for individuals who may contract COVID-19 following contact with infected persons.

The FDA's approval was based on findings from the phase 3 SCORPIO-PEP trial, which showed a 67% reduction in the risk of symptomatic COVID-19 when treatment began within 72 hours of exposure. In this trial, 2,387 participants who had been exposed to a household member with symptomatic COVID-19 were assessed. The results revealed that a five-day course of ensitrelvir significantly lowered the incidence of symptomatic infection compared to a placebo group, highlighting the drug's effectiveness in preventing the virus's spread.

Dr. Frederick Hayden, a noted expert in clinical virology, underscored the significance of this FDA approval, stating that it provides a vital new strategy in the fight against COVID-19. He pointed out that the potential uses of ensitrelvir go beyond household settings, suggesting it could be beneficial in nursing homes, healthcare facilities, and travel groups where exposure risks are heightened. Preventing symptomatic infections in these vulnerable populations is essential for reducing severe outcomes associated with COVID-19.

Ensitrelvir functions by selectively inhibiting the SARS-CoV-2 main protease, an enzyme critical for viral replication. This mechanism prevents the virus from reproducing in the body, thereby lowering the chances of developing symptomatic COVID-19 after exposure. While the drug has received approval for PEP in the U.S. and Japan, it is not authorized for treating active COVID-19 infections in the U.S.

The approval of ensitrelvir arrives at a time when public health experts are advocating for enhanced prevention strategies against COVID-19, especially given the ongoing challenges presented by the virus. Despite a decline in pandemic-related emergency measures, the impact of COVID-19 remains considerable, emphasizing the need for effective prophylactic treatments. Ensitrelvir serves as an important tool in the ongoing effort to protect individuals, particularly those at higher risk for complications, from the effects of COVID-19.
